Patients are eligible to access their medical records. We have provided you with instructions and a helpful form to help us process your request.
State and federal law protect a patient’s medical records, and all patients have the right to request a copy. However, full written approval is required before an application can be processed.
Requesting Medical Records Through St. Vincent
For your information, St Vincent Patient Portal Health System, part of Ascension, has provided a Medical Records Authorization Form that you may download for your personal use. Don’t forget to include the details of the service and the type of information required. If you want the documents sent to you, you will receive an invoice.
Requesting medical records
- There is a flat fee of $6.50 before tax to receive recordings electronically.
- There is a flat fee of $0.90 plus $0.05 per page for impressions.
Medical Record Release Form
If someone requests records for someone else, we require a copy of ID from both parties.
If the patient is deceased, we require a copy of the death certificate and can only issue it to the person listed as the claimant (ID required).
If the patient is under 14 years of age, we request a copy of the parents’ identification document and the minor’s identity document or birth certificate (copy of the identity document).
